It says semester = S, quarter = Q, but an example given for quarter is 4Q.

is it referring to 4th quarter? if so, how about for the semester? does it also have 1S, 2S...etc?

is spring 1S and fall 2S?

Nope
let's say you had English in Semester system and English is 3 credits. so you will have 3S. that number refers to credit.
