EC's:

1500+ of medical research--(1 pub, others projects out for pub)
150+ medical vol
100+ non medical vol
great leadership spots in student orgs
100+ shadowing hours across a few specialties

Thoughts on if I will get accepted into at least one (MD) medical school? I'm applying to lower tier schools and being very realistic. Creighton, loyola, MCW, Oakland, Rosaland, Uniformed, etc.

Any other thoughts besides retaking the MCAT or adding DO schools? Neuroticism?

The GPA-MCAT grid shows your chances for a MD acceptance are in the 20% to 25% range. I suggest applying broadly to DO schools and consider all these:
ACOM
ARCOM
BCOM
WCU-COM
LMU-DCOM
UP-KYCOM
MU-COM
WVSOM
UIWSOM
VCOM (all 3 schools)
NYIT-Arkansas
RVU-Utah

Definitely apply to a wide range of DO schools(but only if you will
attend if accepted). Do you have the MSAR? What MD schools fit your profile. 501 is pretty low for MD actually but maybe its within the 10th percentile for some schools.
