what does "STARS" stand for? is it only errors that are actually dispensed to the patient, or can they be mistakes discovered before dispensing to the pt?

I used to only do it for errors that made it to the patient. Otherwise you’d be pretty busy if you reported the near misses.

basically aren't stars events a way that corporate can fire someone they want out? Like someone who took the 75k sign on bonus 22 months ago and if they can fire them before the 24 month period the person has to pay it back?
