What ARE the most competitive residencies?

I'm not sure you can do a transitional year as your prelim for the competitive specialties. At least I was told you couldn't.
I have no doubt you are correct that the prelim program schedule depends on your program. But the folks I know in prelim years are "the intern" on their teams, and get treated the same.

You definitely can use Transitional, at a minimum, for rads/derm/gas/ophtho/radonc. The Transitional programs I interviewed at had people going into all these fields. Also EM if you match in one of the few programs that don't include PGY-1.

I'm not sure if things changed, or if you met some folks going into specialty programs with different requirements than the norm of their field, but advisors at my school do advise that you are not able to use a transitional program for least some of those specialties.
