What causes LCME to put probation on med school?

I was researching med schools, and I found that Baylor SOM was put on probation until Jan or Feb 2016.
The school still remained accredited, but what makes LCME probate med schools?
 
I would not worry about LCME probation ever unless a school actually loses accreditation like that puerto rico school a while back.

That was awful. We got heart-rending pleas from SJB seniors who were suddenly ineligible for the Match.
 
:'( That's so sad. I would be devastated by that. Do you know if they ever got it back?
Some months later, political pressure caused the LCME to reverse the decision. Many of the SJB Seniors were caught in the Match debacle, though. This delayed them a year...
As the LCME becomes more lax in accrediting new schools, I'm afraid this will become more common.

Ah ok. Wow the SJB students must have had near heart attacks...
