The bare minimum is 2 semesters of Inorganic (General) Chemistry, 2 semesters of Biology (Introductory level), and 2 semesters of Organic Chemistry. Physics preparation is not required for the DAT exam because it is not included on the DAT.
Math and Critical Reading you don't need specific classes, and the same applies for PAT. It's really formal education in the sciences is what counts. The other classes stated (Molecular Bio, Physiology, Genetics, Ecology, Evolution) will indeed help you out, but aren't the core classes "required" to do well on the DAT. It is also assumed that you have had plenty of prior experience with English/Language courses and Math courses.
Personally, I had formal classes in Inorganic Chemistry, Organic Chemistry, Introductory Biology, and Molecular/Cell Biology when I took my DAT exam.
