Thank you for the responses! I was wondering then, what is a reasonable expectation. Can I get to a top 10? If so, what Step 2 score would help with that?
I don't know that there's a step 2 score high enough to get you in alone. It's more than just Step. You need to have a stellar application to get into a top program. That includes step, strong academic standing, research, extracurriculars, and professional orgs (AOA, gold humanism, etc). Not to say that you need all of these but you need have checked at least some of these boxes.
As it stands now, you'd be fighting a very steep uphill battle. You're a quarter of the way through 3rd year with no realistic amount of time to start a new project given clinicals and you're applying in less than a year. Top programs are something you prep for since day 1, intentionally or unintentionally.
The best you can do at this point is kill clinicals and step 2 and get very strong letters of rec from head honchos. Also, start looking into applying for aways in January/February to places that you're interested - doesn't guarantee you'll get the away but if you can secure a decent externship, you can at least show them that you're clinically sound. The caveat here is that you need to really shine when you go. If you know you're not the strongest student then apply to 2nd and 3rd tier programs where you can still get a strong letter.
