What does "activity name" mean?

When it says "healthcare activity name," does that mean I put "volunteer" or "Roadside Physical Therapy" (the place where I volunteered).

Same question with "research activity name." Do I put undergraduate research assistant, or the professor I worked under, or what?

I just used descriptive titles to give someone a quick idea of what each activity entailed. I don't think it's a big deal what you call them, but since there was a separate set of boxes to fill on what the organization is called and where it is, I didn't include location in the title.

Edit: I assume you're talking about the AMCAS application.
