Could you have a pyelonephritis case that presents without flank pain, and has a presentation of just right lower quadrant pain? I remember reading something awhile back about some girl who I thought had an ectopic pregnancy but the question had stated that should work her up for pyelonephritis? This was not USMLE related but hopefully they will include classic flank pain.

You can always have a variety of presentations in medicine, but in terms of the USMLE, pyelonephritis will give you costovertebral angle tenderness.
