Seriously.

If it were "Doctor of Medicine", wouldn't the abbreviation be DM?

And "Medical Doctor" just sounds silly...
 
The order of the letters seems reversed because it's originally from Latin, where adjectives and other modifiers usually come before the noun they modify. It stands for Medicinae Doctor, which means doctor of medicine.
 
just like a PhD means Doctarate of Philosophy but the order is still PhD not DPh.
