Nothing to be worried about. I got something like this from UMich when I applied and I ended up getting an acceptance. They had a question about when I took the DAT and my Bio score because there was some kind of problem they were having at the time. (They did end up fixing the problem though and got my full application)

You shouldn't be worried. This email means that they are heavily considering sending you an invite and want you to clear something up before they send you the invite. Luckkkky 😛
