What does "witness" mean on a form?

Sorry if this is a stupid question, but I am confused about UCSD's Technical Standards Certification Form.

It has the usual "Print Name", "AMCAS ID", "Signed" and "Date" for you to fill out, but it also has a "Witness" part. What are you supposed to put for that part? Is someone supposed to witness you signing their technical standards certification form?

I'm not familiar with the form, but generally if there's a witness line it means someone is supposed to witness you signing the form and that you are certifying that it's true. It's a legal formality -- makes it harder for you to say you didn't read or mean to sign the form if you end up contesting it later. You can never be both the signer and witness.
