What if you know you want to become a DO, not just a physician, but an osteopathic one, and you are so eager that you apply only to DO program and even mention it in your personal statement. Then, August rolls around and you are like, hmm, may be i should apply to like 4-5 allopathic schools, and you do, what do you do? contact the do program and tell them? just not mention it?

You don't need to tell them anything unless they ask. Even then you don't need to tell them if you don't want to. It will really be up to you.

Timbo0984, you don't have to worry about this. Don't even mention this to the D.O. programs unless they ask. The applications for allopathic and for osteopathic are separate, and they wouldn't know what allopathic schools you're applying to.
