What is on the primary application besides personal statement?

Hey all,

I am trying to figure out what questions are on the primary application so that I can work on it beforehand.
I know that there are:

Personal Statement (4500 characters limit)
Manual Dexterity (600 characters limit)
Any relatives in dental field (600 characters limit)
Honor code violation/felony stuff (600 characters limit)
Award/honors (175 characters limit)
Academic Enrichment (175 characters limit)

Could anyone tell me what else is in it? I've heard of things like Leadership, research, and shadowing, but I am not positive..
 
Awards / Honors / Scholarships does not have a box to enter a description, only name/sponsoring organization/date.
Dentistry / Shadowing Experience (175)
Extracurricular/Volunteer/Community Service (175)
Work Experience (175)
Research Experience (175)
Academic Enrichment (175)

Is Extracurricular/Volunteer/Community Service all included in 175 characters, right?

Also, for tutoring financially disadvantaged population through an organization (I get paid) will be counted as work because I get paid or will it be categorized as academic enrichment?

No, you have room for 5 activities per category so 5 for Dentistry / Shadowing Experience, 5 for Extracurricular/Volunteer/Community Service and so on but each individual activity has its own 175 character limit for description.

I'd list that under work experience because you get paid.
 
I can't believe I do not have any manual dexterity experiences. The only thing I can think of is Super Smash Brawl district champion lol.
 
Can these be any kind of awards and honors such as sports - tournament placements? or coaching certification from the USA Volleyball?

The instructions are:
The purpose of this section is to highlight any special accolades you have received. List awards, honors scholarships in order of importance to you. You may include awards, etc. from high school and college, but keep in mind that college accomplishments are given higher priority by many admissions committees.

...so pretty broad but in the end, will something like that determine whether or not a school admits you.....probably not.....so up to you.
