I've been searching the DO forums for "X school vs Y school" threads, and I've been seeing the term OPTI come up many times. I know it stands for Osteopathic Postdoctoral Training Institution, but I can't find what it means apart from what the letters stand for. Is it certain residencies that a school has that is guaranteed to its graduates? Please someone fill me in!

Sort of. They are residencies setup by the school's Post Graduate Education department. They are not guaranteed for it's graduates but as my Dean carefully put "OPTI helps us establish rotations for our 3rd and 4th years, which help you get facetime with the residency directors, which helps you land those residency spots. So while you are not guaranteed a spot, it will increase your chances if you rotate in the same location."
