What is the proper documentation for shadowing?

Do I keep a spreadsheet with hours, physician specialty, contact info etc.?
Do I need signatures, letters, or anything else from the physician?

You can document your hours any way you like. They're for your own personal bookkeeping. I personally use excel.

You don't need any proof or validation of hours. It's on your own honor. Of course, you provide schools with contact information, so they can always verify the hours if they're suspicious.
