This might be a stupid question, but what do people usually do after they finally finish their MD/PhD? Residency? Post-doc? Both? Neither? Can you practice w/o doing a residency? Or do research w/o a post-doc?

most people do a residency. there are some fast track internal med residencies where you finish more quickly with clinical stuff and have dedicated research time. i don't think you can get away without a residency if you want to practice medicine clinically.
if you just want to do research, you can probably go into a postdoc, but honestly that leaves you much fewer options than if you just went into a short residency and finished boards certification. but really there are various options.
many fields of medicine have research opportunities as part of the residency also.
