I have found a doc to shadow, but when I do, what info should I request from him? I know that I should ask questions about optometry, but should I also ask for his contact info so that I could have proof that I shadowed him?

The only proof I ever needed of shadowing was the address of the OD I shadowed. Just ask the OD what you really want to know about optometry. Especially ask him/her about their mode of practice and what they like about it. There isn't anything specific you have to ask. It is for your benefit.
