What should I do?

Background Information: I was raised in Detroit Metro Area and moved to a new state when I was in the 10th grade. I'm still a resident of this state as my parents still live here. I'm currently attending a university in NYC (debt free). Regarding the state I am currently a resident for, I just do not vibe with it all. It's a different culture, and I didn't enjoy my time here in high school from Grade 10-12 at all. I feel that returning to the state for medical school will be even more difficult after spending the past 4 years in NYC.

Currently, I have two acceptances. One from my unranked state school (I'm not going to name it as it has very little SDN presence) and one from Wayne State University. At my state school, I can graduate with 0-20K in debt, while at Wayne State, I will be in around 140K in debt considering I receive 0 dollars in aid. I would say Wayne State has a better reputation. My state school also has an A-F grading system, and an old curriculum. It is 3 hours away from where my parents live. WSU has recently updated their curriculum to being systems-based and has a P/F grading system. I have friends who attend the school and they all speak very highly of it.

Contrary to my current home state, I really like Detroit. My brother still lives here, thus I will be close to him if I attend. I would have no problem staying in the area for my residency.

Would I be committing a mistake by turning down the opportunity to graduate debt-free from my state school and instead going to an okay school like WSU where I may have to accrue debt upwards up to 140K? I am not really sure what specialty I am interested in, but right now, I'm definitely leaning towards a non-surgical field. I am on the waitlist for some top 30 schools, so I may have some better options, but I just want to make sure I'm not being completely silly.
 
It sounds like you already have your mind made up. 140k is not very much in the grand scheme of things, but attending school for (basically) free is also a very real benefit. You don't have to make a decision now, but I would press Wayne state for financial aid. It sounds like that would make your decision a slam dunk.
