What should my Friend do to boost his app(4x reapplying)

He has strong stats, but lacks Ecs. Here's his app basically:
MCAT: 527(131/132/132/132)
Gpa: 4.0 for both sgpa and cgpa
Endocrine research with 5 pubs as first author
Health care activities: none
Volunteering: none
He's applied to all TMDSAS schools and has not gotten a single II in the 4 times he has applied. He doesn't want to do any volunteering or anything related to health care b/c he feels that volunteering is a waste of time and health care activities are boring(except being a doctor, according to him..). He's set on becoming an MD, but doesn't wanna do volunteering and health care activities to get there. What schools should he apply to that don't look for volunteering or health care activities but just care about stats?? He's considering the Caribbean (that is, if they don't require volunteering and health care activities).

The reality is he will need those things as part of his application.

Why on earth is he interested in being a doctor if he doesn’t care about doing health care activities or philanthropy?

Research... oh never mind.

Then they should go apply to the Caribbean. With 4 strikes against, it is clear this candidate is unwilling to learn or adjust. Lacks any service orientation. The lack of progress as a reapplicant would probably black-ball him (my programs would). In my opinion, the door is closed for US until the attitude significantly changes.

If you don't learn to play the game, you will never win.

He should just get a PhD. If he doesn't want to do healthcare activities, how does he know he even wants to be a doctor? By the sound of it, he seems more interested in research rather than treating patients. Just my 2 cents.

99% chance this is a troll post. There is absolutely no way someone is smart enough to get a 527 MCAT score and yet is stupid enough to not learn the same lesson 4 times.

You friend doesn't want to be a doctor. The Carib is a good place for him

Your friend should try nursing, they are now getting paid more than FP and Pediatricians.

This is why the applicant is DOA. Adcoms do their best to screen out arrogant fools. It's surprising that he didn't just learn this his first time and come back again just having checked the freaking boxes for those. Just having the boxes checked would mean he's got an excellent chance of getting into an MD school.

As for the Caribbean Crapshoot, it's ill-advised. Guy should do research or something other than medicine.
