... Propanolol is wrong for Thyroid storm.. ? it's IV Esmolol? ... two different Step 2 Review sources differ.

What is correct by the licensing board ? Am I missing something>

According to UpToDate, Propranolol is listed first with Esmolol as an alternative. Was there a question asking you to choose between the two?

Thanks for that.. I just found a discrepancy amongst the review sources (you know, about these sources for Step2 CK review..) ... looks like there IS IV Propanolol... what the heck.. but acute things being acute, I figure it might be import.

I would pick propranolol. IV esmolol has a very short half life, less than 15 mins IIRC.

I'm hoping they still aren't testing second line, because this would only be a minor example of the difficulty, of course we don't give propanolol in asthmatic..

BTW, it's the same idea with the IV or oral PTU... I think headed for the ICU anyway, so what's with all this oral meds for thyroid storm..? anyone have any insight?

Meanwhile the patient question is probably about ACLS indications for intubating, so none of the drug choices will be correct.. I read once that if you see that the patient question has listed <10breaths a min .. they are asking about intubation.. (even though I couldn't find anything in the ACLS guidelines that gave a bpm cutoff, but rather a qualitative assessment rather than a quantitative)
