What to add with Supplemental Fee

Hey so this might be silly, but are you guys submitting the supplemental fees with just a check (and anything else they request)? Or including a letter? Just checking.

Wrote my AADSAS ID in the memo section. One school indicates that they wanted an index card of my personal contact info with my check. Pay close attention to what they ask for.

I printed out my AADSAS # on mini return address labels and placed them in the 'memo' section of the check

... idk. It just seems empty. Last year I did index card with my contact info. but I don't want to seem unprofessional

On my envelopes, I made sure to have the addresses printed on. Idk if your printer can do that but I just felt it looked more professional too.
