I am gonna bring my application, note pad, and secondary to my Midwestern interview. Question: during the interview, can I look up my notes to answer the interview questions or can i read off the note to ask question to the interviewers????

Don't need to bring anything really. A padfolio/pen is optional.
I can't tell if you are joking about the looking in notes to answer questions. If you aren't, that would be a reason to get rejected.

I don't think it would be too terrible if you had some questions about the school for the interviewer but needing notes to answer a question sounds like a bad idea.
