If you're worried about the match should you rank a prelim program higher on the prelim list because it has the chance to get you into the radiology program at that site for the next application period? Or just rank the intern years (transitional, prelim) as you like them and hope for the best? Any advice?

If you are worried about not matching you may 1) want to go somewhere connected or prestigious where connections may help next time around or 2) stay at the home institution to remind them to help you next time around.

If you think you will match you can rank your prelims/transitional for each rank on the the radiology rank list separately. I was in that boat and I went for a nearby place that I knew was decent and where I was sure to match so my wife could start looking for a job before the match. Many people in this boat also stay at the home institution to limit themselves to just one move...

You mentioned on the prelim list. Wanted to make sure that you actually meant putting the prelims/transitionals at the end of your primary list vs. your supplemental lists for each radiology program. If it comes to this point, you will have officially not matched into radiology. Then, I would think the next best course of action would be to see to it that it doesn't happen next year. Improving your chances would be all I would think about at that point. I think best case is to do transitional at home institution. No funding issues compared to prelim, more time to do stuff like research to help, and your still around the home folks.
