What to document on shadowing hours

I have started an excel sheet for my shadowing hours. Not 100% sure what to document; I have the date, doctor, office, summary, and hours. I don't really know what to put in the summary section but I thought it would be beneficial to note effective things I saw, etc.. Am I missing anything?

That seems good! On the application you just need to put the Dr's name, address, the date you started shadowing, the approx. number of hours you have, and a brief 600character description, which I think you should be easily able to write 🙂

If you shadowed at a dental clinic, can you put the name of the clinic instead of the names of all the dentists you shadowed?

I personally have a spreadsheet and put the clinic and the individual dentist. Always better to have more information than less.
