What to provide in Email to LOR writer?

Hi everyone!

This might be a dumb question but...

A physician I've worked with has agreed to write a LOR for me for residency. He told me to send him my CV and how/when to submit it. My school hasn't given us access to ERAS, so do I just tell him to hold onto it for now until I get access to ERAS and can send him the link? Also he didn't ask for my personal statement, and I haven't written it yet...should I just tell him in the email which specialty I'm planning on applying to (I'm applying DR, but told him I was thinking DR or surgery when I rotated with him)? What other information would he need to write the letter?

I would give him some sort of personal statement or outline of one. Doesn't have to be remotely close to a finished copy but just something to highlight that you are going into DR, why you want to go into DR, etc...
