I don't understand, your medical school doesn't have a IM program (residency) or cardiology program (fellowship)?

In any case, concentrate on matching into top tier IM residency first, which will make matching into cardiology fellowship easier. Research productivity is more important as this will result in stronger LoR. I would recommend joining the lab that can get your name into papers.

Sorry I wasn’t clear. It doesn’t have a IM program.
 
Go for the top tier position. IM programs don’t care what you do your research in as long as you’re productive. Half the applicants my program interviewed this year did their research in surgical specialties. It won’t affect your fellowship application to cardiology because fellowships don’t really care about the research you did in med school either. Hell, I did half my research in GI as a resident and matched cards.
