I think the full Bate's text book is the gold standard out there. Don't know much about the pocket.
What does fit in a pocket is a phone. Multiple ways to get Bate's on there if you want some heavy reading. However, despite my school having a pretty decent PE website, I like this one from UCSD better:
http://meded.ucsd.edu/clinicalmed/vital.htm. They do a very good job of explaining every little bit of the exam and the reasons behind each step (you can see them using cadaver models to show exactly where the radial pulse is, etc). This site could easily be accessed by your phone/labtop/tablet.
