any non-nurses have ideas.😴

In the absence of physical symptoms along with psych symptoms, I'd go with therapy and/or a dog. 😀

There is no best antidepressant. What matters is how useful a certain one is to the patient.

Yep. And prescribing a certain drug is but one part of the therapy.

Exercise, Socialization, and Sleep.

THEN, if that all doesn't work...I might consider psych drugs.
Thanks a bunch guys, ya'll answered my thought processes. 👍

wellbutrin works great for me.

I agree, it is a good one
Pros:
no sexual side effects (that I am aware of)
more energy- esp good for the winter blues
helps with concentration, can alleviate sx of add/adhd
helps with carb cravings, actually can shed a few pounds on this med
and if you are a smoker who wants to quit, this med is amazing! (Cigs taste absolutly disgusting)

Cons
having to take a pill daily
can make anx sx worse in some cases
my friend had to stop taking it d/t a rxn- gave her hives? but haven't heard this from anyone else

Other than meds, physical activity (even the non lay kind) is a great antidep! Just getting outside and breathing some fresh air everyday works great for me. I find that having someone to talk to is also very helpful, I don't think I could get thru med school without this!

Good luck to you 🙂

In all seriousness, the gym does wonders

In RCTs, all antidepressants have have been proven to be more or less equally efficacious. But you can never predict how each individual responds to each class (or drug within that class, for that matter).

So, literally, pick one. Preferably one with a low side effect profile.

St. John's wort is OTC and has been rather firmly established as effective for mild to moderate depression by Cochrane reviews. I haven't tried it, but I've always been really curious.

Yup, all the SSRI, SNRI and TCA have similar efficacy in studies, but patients vary in response and side effect tolerance.

You might not start exercise, socializing and sleeping until you start meds. What do you do, if your exercise, socialization and sleep rule doesn't make the patient socialize, work out, and take naps?

Mirtazapine to me seems like the best one for a med student, as it has good efficacy on ocd symptoms, social phobia and generalized anxiety as well.

Actually a meta-analysis published this past year shows that they do not all have equal efficacy.

http://www.ncbi.nlm.nih.gov/pubmed/19185342

Interesting.

In terms of response, mirtazapine, escitalopram, venlafaxine, and sertraline were more efficacious than duloxetine, fluoxetine, fluvoxamine, paroxetine, and reboxetine. In terms of acceptability, escitalopram, sertraline, citalopram, and bupropion were better tolerated than other new-generation antidepressants. These results indicate that two of the most efficacious treatments (mirtazapine and venlafaxine) might not be the best for overall acceptability. Here, we did not investigate important outcomes, such as side-eff ects, toxic effects, discontinuation symptoms, and social functioning. However, the most important clinical implication of the results is that escitalopram and sertraline might be the best choice when starting a treatment for moderate to severe major depression because they have the best possible balance between efficacy and acceptability.
Thanks for the reference.

its interesting to note which threads stay open, and which get closed.

The OP asked which antidepressant medication is best to treat a medical student, with minimal side effects such as loss of concentration and drowsiness, so as not to interfere with said high functioning smart individual.

How much of an affect would you say. Ie. recommend when someone is contemplating suicide, or mild depression? does caffeine improve mood at all?

Actually I didn't say anything about it being a medical student, it could be a nurse I was talking about. (okay that's a bit far fetched...ha ha)

keep it coming guys!!👍

Beats me. It improves my mood and helps keep me productive and focused. It's also cheap and over the counter . That's about all I know.

Most of the pharm students use wellbutrin or fluoxetine...I would personally use wellbutrin..as long as you dont have a seizure disorder. Best thing is to talk to your doctor about it as advice on the webz is not smart.

Most SSRIs are actually stimulants and not sedating though it is different for every individual. Sertraline is usually one of the first line meds but buproprion isn't going to have the sexual side effects. Escitalopram is also a good drug but expensive.

Caffeine certainly improves mood if you haven't had it in a couple days and then have some.

If you're depressed, seek a psychiatrist, and they'll give you the proper answer to your question.
