Oh...might depend on how well established the program is. I'm part of an inaugural class so we had to go through all kinds of hoops to get our logo approved and so on. But we have done everything ourselves - choosing the clothing, colors, styles, and so on. Faculty didn't do anything and the school itself was who approved the logos. It happened because we wanted stuff, so we took it upon ourselves to make it happen - it didn't come from faculty or the program.
