When to apply for AOA first-year approval?

Does anyone know when it is that one applies for AOA internship approval for an ACGME residency (section 42)?
Before the internship starts, during, or after completion of first year?

Also, how long (generally) does the approval process take?

OK, I finally found it. For anyone interested:

You can do it before or up to 90 days from starting. Or even during, or after (res 52).
