When to do residency with MD/PhD

What is the timeline for an MD/PhD program with residency after? What I mean is, if you start MD and PhD at the same time (such as a combined program) would you start residency after getting the MD or wait until the PhD is completed as well? I think this wouldn't be too hard if you are able to get a residency spot at the same school you are doing PhD work, but does anyone have any experience/ advice concerning this?

Midlife is right this isnt the place.

All the MD/PhDs at my school do 2 years of med school, take step 1, then go out and do their PhDs, then come back finish the last 2 years of med school, and then enter residency.
