when to order counseling?

I am taking Step 3 soon and have a quick question - that I couldnt find answered by first aid or this forum so far:

WHEN DO WE ORDER ALL OF THE COUNSELING?

Is it when the pop up box comes up at the end of case saying we have a couple minutes left?
then we type in 'counsel' and put all of the appropriate orders...patient counsel, drive with seat belts etc and schedule follow up appt


Or if I have a smoking patient coming in with COPD exacerbation- should I counsel right off the bat- and order smoking cessation and patient counsel
