when to start maintenance dose after loading dose

When should maintenance dose be started after a loading dose. If loading dose for nexium is 80mg, then maintenance drip at 8mg/hr, that's 80mg every 10 hours. Do i start the drip 10 hours after the loading dose or can i start the drip right after the loading dose?

Start the maintenance drip as soon as you finish the bolus.
 
In most cases, if it is a continuous IV infusion drug, the main. dose starts right after bolus.

Thought provoking question (BTW this is a valid question)

Why on earth are PPI continuous drips done?? PPIs are irreversible inhibitors and thus just need to strike once, and are good for at least 24 hours. what does a drip accomplish?

Guidelines say otherwise in GI bleed, playa. And until they amend the guideline...you follow the guidelines. Though you might be right. Later and smaller studies have shown little difference in continuous vs intermittent dosing. But if you deviate from guidelines, lawyers come a-knockin'...
