Do I write all of my extra-curricular activities (student govn't Intramurals etc.) under additional information, Answer #15?? Is that also where I list all of my experiences in shadowing M.Ds and D.Os??

i think you should just fill out the extracurricular activities.... and leave the additional information in case you can't fit all the activities in the space given.
