Where to find optometry patient file templates?

I'm looking for patient file templates in PDF form (ie. to take down case history, acuities, motilities, plan, assessment, etc etc...)

I've looked high and low online but I couldn't find any PDF templates of patient files for optometrists.

Can someone show me some example or point me to the right direction?
