Which entity is responsible for ranking residents?

Quick question for anyone in the know:

For med school admissions, they get your app, offer an interview if they like your app, your interviewer writes up their opinion, and then the "admissions committee" makes a decision to accept/reject/etc.

For residencies, who makes that final decision for rank order of interviewees? The program director? A committee?

Different programs do things completely differently. Some places have a faculty meeting where applicants are discussed with or without resident input. For others one person (chair or program director) makes the list unilaterally. Some use more formalized systems (scores, etc), others do not.
