I am planning on pre-studying and I want to know which course is going to be tougher out of my second semester courses for M1 year. I have four total courses in my second year and I want to pre-study the hardest course to prepare myself

Are you taking these courses concurrently or in order?
 
Dermatology is easy. I think Musculoskeletal is the hardest. I hate learning anatomy. Hematology can be hard if you dont know anything. Microbio is a lot of memorizing but is easy otherwise.

I think if you went through pathoma a little bit that is a good learning resource for solo studying.
 
This is going to be school dependent. Ask students in the years ahead of you what the courses are like and what is feasible to "pre-study". That being said, I'm not sure how often pre studying in med school for specific classes actually helps anyone, vs giving themselves a break to refresh.

I get taking breaks, but I am just not in a position to give myself too much of a break. I'm remediating the entire year and if I fail another course in med school I'll probably get kicked out. I am basically in a position where I have to pre-study the hardest course and cross my fingers on the other courses
This is impossible to answer because it depends upon the professor's teaching style and content and your learning style.
My exams are almost entirely NBME, aren't some NBME exams a lot harder in certain subjects compared to others? Like I have heard that cardiology tends to be every med student's nightmare. I have also heard that Nervous System can be difficult as well?

My students ace our Neurology course.

Hard or easy is going to be student-specific. We're not going to be able to tell you what you want to hear.

Rather, you should be optimizing your learning to make sure you don't make the mistakes of last year.
