Which general surgery programs to apply to?

I'm at a loss to what programs to apply to and how to choose them. My school is relatively new, so faculties are not as up to date.

MD school (new-ish/bottom tier)
Step 1: 246
Step 2: 258

2nd quartile in class rank
Only 'pass' surgery clerkship (but did honor IM)

2 pubs (one 1st author)
5+ abstracts/presentations

Not sure on academic vs community (will apply to both, I think), hence also need help with knowing which are the good community and academic programs to also apply to
