I am a 3rd year med student currently attempting to get research experience to improve my resume and ability to get a good residency. My question is which matters more to a residency director and my hopes of getting hired.

Published Case report vs unpublished poster presentation of actual research

I am currently working on the case report, but I know it's just a case report and definitely doesn't hold the same honour as real scientific research. I don't really have time in my 3rd year for real research (nor am I enthusiast about it). I may also be able to take part in a poster presentation in neurology. However this poster won't officially be published but I may put it on my resume. Which improves my resume the most in case I only have time for one?

It depends on the director. But before you go too far I recommend you read my post number 6 in the thread "Trials, evidence based practice and research" on case reports so that you are aware of their value and pitfalls. Case reports are useful in that you will one hopes have described some unusual occurrence and if it's done well that will be in your favour but get someone well qualified to read them to make sure you have made no obvious blunder though hopefully this has been done if you published.
