Which Letter of Recommendation Should I Use for AMCAS Application?

I'm applying for the 2019-2020 AMCAS cycle and have 5 recommendation letters so far: one doctor, the PI for my research lab, the post doc for my research lab, one of my biology professors, and one from the Honors coordinator/advisor for my major. I want to get one more LOR, but I am stuck between my volunteer coordinator at a hospital I have volunteered at for 3 years and a D.O. that works in the Rush University Medical Center ED (will be shadowing this doctor for about 20 hours total). The only problem is that I think my volunteer coordinator uses a common format for LORs for everyone who asks for one - saying this person put in "x" amount of hours and is a great worker blah blah blah. Would it be better to diversify my LORs with a volunteer coordinator (at the expense of an already formatted LOR) or to just get another LOR from the doctor at Rush. Any input would be extremely helpful!
 
Shadowing letters are extraordinarily useless (for MD applications).
DO schools do love a DO letter, though.
I have an MD letter for 7 of my schools that all ask for a clinically relevant letter.

He is also my boss at work, but I think the shadowing is what he talked about.
