Which Schools Offer an Master's? A B.S.? A Ph.D.?

As I'm looking through the United States's colleges of optometry, I'm noticing not every institute offers degrees beyond an O.D. Does anyone know (or, perhaps several contribution will complete this list) which colleges offer a joint Master's/O.D. program? Which offer a joint Ph.D./O.D. one? Which do the opportunity to earn a B.S. alongside an O.D.?

SUNY has the option for a joint OD/MS or OD/PhD in Vision Science.
