Who is in charge of counting up your volunteering hours

Who is in charge of counting up your volunteering hours? I am planning on volunteering in an environment where no one will be residing over me to determine how much volunteering I do. Furthermore, do we have to report our own volunteering hours on pharmcas and on the supplemental application, or do we have to get someone to do it?
