I know this will be a silly question:

But why is that light at the end of the tunnel a train? A lot of work in residency?

It's a joke, young fledgling. You try as hard as you can to get out of residency to reach the promised land of milk and honey only to find even harder work. Hence the light at the end of the tunnel is a train.
