Can any MD/DO give a note/script for windshield or does it have to be ophthamologist? What are the steps required for someone who has trouble driving at night due to other headlights and during day with reflections off other cars?
In the US, probably dependent on state regulations, of which there are probably 55 different sets of regulations because of places like DC and Puerto Rico.
