I don't know why, but I'm having the hardest time deciding what to write. Can you guys tell me in what form(email, letter, phone call) did you guys withdraw after your acceptance and also what you said? thanks a lot!

Just send them a convenient e-mail saying I am no longer interested in attending XYZ Dental. Thank You for your acceptance, and for taking the time to review my application, blah, blah blah. It really isn't that hard. You don't need to please the DS anymore, you are already accepted, so don't loose sleep over it.
