Lets patients know that it’s not all just fun-and-games sleepy time and that they might want to care about who is at the head of the bed…

Let me share my decades of experience with you on this matter. Many patients, like Joe Biden for example, have mild dementia or early signs of cognitive dysfunction. Many studies have confirmed this fact prior to any operative procedure. Then, even PROPOFOL IV may worsen such cognitive dysfunction for a select group of those people.

For example, I had a 83 year old man with moderate dementia for outpatient surgery. He was jus slightly more advanced than Biden. He received propofol 30 mg IV x 1 for the brachial plexus block and light sedation of propofol, 30-50 ug/kg/min for the case, with no other medications besides the Cefazolin. His wife reported to me the next day that he was more confused than usual and it was 1 week before he returned to baseline.

You really blew my last post way out of its intent. I have no malice against Joe Biden and the patients with mild POCD are very similar to him quite often.
Please note I used the word "mild" which is in all honesty the truth. My point was that propofol is still the best choice for sedation that we have for this group of patients but even then, there is no guarantee of outcome postoperatively. Some may still have worse cognitive function but it may be temporary from baseline.

You don’t diagnose dementia so it could be forgiven except you should know better but mci/mild/mod dementia are actually diagnoses that have defined criteria established by outpatient neuro cognitive testing rather than a gestalt feeling applied by a provider.

Don’t mind Blade, he has some mild cognitive impairment. For his anesthesia I would recommend a block, no benzodiazepines, and +/- a light propofol infusion.

If sedation hastens dementia, then lack of it should slow it down or reverse it. I suggest biting down on a leather strap for all ortho procedures, demented or not.
