When the Work/Activities section asks for "contact title", do they want me to say Dr. ______ or like their role in relation to me, such as "supervisor"?
You can use some judgement in filling out this blank. For example,

-if you want to give the title of the head of your lab, you could enter PhD, or Chair, Biology Department, Researcher, or Principal Investigator. [I'd probably pick the last choice as most descriptive and most relevant to the activity "Research".]

Use the title relevant to the activity. But you can also sneak in the true professional title if you like.

For example,
Contact's First Name: Professor John
Contact's Last Name: Smith
Contact's Title: PhD, Faculty Advisor

This will print out as Professor John Smith, PhD, Faculty Advisor

Or,
Contact's First Name: John
Contact's Last Name: Smith MD
Contact's Title: Chair of the XXXX Committee

This will print out as John Smith MD, Chair of the XXXX Committee

Feel free to give a more explicit example if I haven't answered your question completely.
