WVU Secondary Banner application?

For those of you who received the secondary application materials from West Virginia, one part of the required items is a so-called 'Banner Application.' Did anyone else find any links or info about this item? The link they send to you after the initial screening has nothing about anything related to a WVU banner application other than that you're required to send it with the rest of the secondary. The office staff at student services seems very unreceptive about sending you anything other than the link that has a non-existent banner app. Anyone else have this problem?
