z-code modifier and technical assessment requirement?

Are participation in MolDx program, submission of a Technical Assessment, and application of Z-code modifiers requirements, if the performing laboratory is in the northeast Novitas jurisdiction?

Or is the more important factor whether the hospital client bill is located in a relevant MAC jurisdiction, e.g. Palmetto.

I had thought the answer to the above questions would be NO, as the requirements are all based on where the laboratory performing the test is located.

The answer is sort of yes. If the lab is NOT in the jurisdiction, typically they don't have to submit to MolDX, but if they are client billing another lab that is, they do need to register and get a TA.
